New-age fires: On India’s ICU fire accidents
New-age fires: On India’s ICU fire accidents
Fire (आग, अग्निकांड) accidents in ICUs need better and more evacuation (निकासी, सुरक्षित बाहर निकालना) facilities
Not too long ago (कुछ समय पहले, हाल ही में), India’s fire accidents typically (आमतौर पर, सामान्यतः) occurred (घटित होना) in industrial facilities (a place, amenity provided for a particular purpose; establishment, center, building, premises), large offices, mass gatherings (बड़ी भीड़, विशाल जनसमूह), firecracker units and railway compartments, caused by faulty machinery, weak safety procedures and careless use of flammable (ज्वलनशील) materials. While these risks (जोखिम, खतरा) persist (बने रहना, जारी रहना), what has got added to (बढ़ाना, वृद्धि करना) the country’s poor (of a low or inferior standard or quality; substandard, bad, deficient, inadequate) record in fire safety in the last 15 years or so is residential buildings, hotels and hospitals, especially Intensive Care Units (ICU) (गहन चिकित्सा इकाई, गहन देखभाल कक्ष). Electrical fires now seem (प्रतीत होना, लगना) predominant (प्रमुख, सबसे अधिक प्रभावी), often (अक्सर, प्रायः) caused by increased appliance (उपकरण) use, overloaded (burdened with too much of something (such as electrical current); overburdened, overwhelmed, excessively loaded, overstrained) electrical systems and poor maintenance. ICUs are especially vulnerable with their oxygen-rich environments. This month, back-to-back accidents at neonatal (relating to newborn babies, especially during the first 28 days of life; newborn-related, postnatal, infant, newborn) ICUs in Amravati, Maharashtra, and Chhindwara, Madhya Pradesh, claimed ((of a disaster or accident) cause the loss of (someone’s life); take, cost, cause the loss of, result in the death of) the most vulnerable: (individuals who are most exposed to danger or harm and least able to protect themselves; the most susceptible, the most defenceless, the most at risk, the most endangered) newborn babies. The Amravati fire was put out (extinguish something that is burning; extinguish, douse, quench, smother) in 30 minutes, but the smoke (fumes, exhaust, vapour, smog) was too much (excessive, overabundant, disproportionate, unmanageable) for the three babies admitted because of complications (complex medical issue). A faulty ventilator (a machine that provides artificial ventilation by moving breathable air into and out of the lungs; respirator, breathing apparatus, mechanical breather, life-support machine) is suspected (believe or feel that (someone or something) is guilty or responsible for something, without certain proof; believe, think, surmise, guess) to be the cause. In the NICU at Chhindwara District Hospital, a short circuit in a warmer triggered a fire and three babies, just days old, suffered burns (an injury caused by exposure to heat or flame; scorch, scald, singe, sear). Over the past 15 years, ICU fires, including the one in Jhansi in 2024, in which 18 newborns died, have not served as (function as, act as, do the work of, operate as) a wake-up call (an event or situation that serves as a warning that action is needed; warning, alarm, alert, cautionary sign). The Amravati fire shared many features (a distinctive attribute or aspect of something; characteristic, attribute, quality, aspect) with previous incidents.
Perhaps (maybe, possibly, conceivably, it could be) the best way (method, approach, technique, system) to respond to these new-age (modern, contemporary, current, latest) fires would be to treat ICU accidents as a distinct (different, separate, individual) category (class, group, classification, type). ICUs are sealed (closed securely so that nothing can get in or out; airtight, closed, fastened, secure) compartments where smoke fills up very quickly. In an NICU, where patients cannot self-evacuate, evacuation drills are essential. A best-case setup would have three independent exits (way out, door, egress, escape route), with at least (not less than; at the minimum, minimally, no less than, fully) two allowing horizontal (parallel to the plane of the horizon (here, moving patients on the same floor level rather than down stairs); flat, level, plane, parallel) evacuation, automatic sprinklers (a device for spraying water automatically to extinguish a fire; fire sprinkler, water sprayer), independent power lines for large equipment, oxygen cylinder bed supports, and fire drills (a practice of the emergency procedures to be used in case of fire; emergency exercise, evacuation practice, safety rehearsal) every two years. A well-designed (logically constructed, carefully planned, expertly crafted, structurally sound) ICU also locates oxygen outlets (port, socket, connection point, terminal) away from sockets (electrical outlet, plug point). Following a Delhi NICU fire, the National Neonatology Forum (a premier, highly influential professional organization of neonatologists and pediatricians in India. Established in 1980, it has grown to include over 10,000 members dedicated to improving newborn healthcare, research, and advocacy) called for mandatory fire safety certification for NICU-equipped hospitals, regular power audits, analogue addressable alarms, and an evacuation protocol — now RACE (an emergency fire response protocol acronym standing for Rescue, Alarm, Confine, Extinguish/Evacuate, widely used in healthcare settings to train staff on immediate actions during a fire) or Rescue (save (someone) from a dangerous or difficult situation; save, save the life of, come to the aid of, deliver), Alarm (to trigger a warning sound; alert, sound the warning, signal danger, raise the alarm), Confine (keep or restrict someone or something within certain limits of space; restrict, enclose, limit), Extinguish/Evacuate. The 2026 National Guidelines on Fire and Life Safety in Healthcare Facilities can be a useful starting point (beginning, basis, foundation, origin) for ICU-NICU safety. Beyond (apart from; further than; apart from, other than, outside, besides) all this lies ((of a problem, issue, or quality) be found to exist or reside in a certain place or situation; exist, reside, be present, be found) an issue (problem, matter, concern) in power quality: electrical harmonics. NICU equipment is packed with electronic components that draw distorted current, which can silently overheat neutral wiring (the electrical conductor that completes the circuit back to the power source, which can overheat if harmonic currents are present; return wire, zero-voltage wire, neutral conductor), transformers, and loose connections (faulty joint, unsecured wiring, poor contact, weak electrical link) without tripping ((of an electrical switch) to automatically turn off the current when a fault is detected; disconnect, cut out, switch off, break the circuit) a breaker (circuit breaker, fuse, safety switch, trip switch). Post-fire (occurring after a fire; subsequent to a fire, following a blaze, after-blaze, post-combustion) reports citing generic (general, common, non-specific, comprehensive) “short circuits” or “technical faults (mechanical error, system failure, technological breakdown, equipment glitch)” may well be missing this harmonic-driven (caused or propelled by electrical harmonics (current distortions); electrical-distortion induced, waveform-irregularity triggered, power-anomaly caused) degradation (deterioration, breakdown, decay, degeneration), a largely (to a great extent; on the whole; mostly, mainly, primarily, principally) unaudited (not having been officially examined or reviewed; unexamined, unchecked, uninspected, unverified) risk factor.
