The National Testing Agency (NTA) has issued the hall tickets for the National Eligibility cum Entrance Test (NEET) and self-declaration form at the official website neet.nta.nic.in. The exam conducting authority, NTA will conduct on September 12, 2021, taking all precautionary measures to contain the spread of COVID-19.

More than 16 lakh candidates will appear for the NEET-UG exam and to reduce the risk of spreading COVID-19, the NTA has issued a health-related ‘self-declaration form’ to ensure that each aspirant is safe to sit for the test. NEET 2021 Admit Card: How To Fill Self-Declaration Form:
The details which must be filled in by the NEET aspirants are:

Declaration by candidate

If aspirants have experienced any of the flu-like symptoms in the last 14 days, then tick whichever is correct

Write the name of the city or country in which the candidate has traveled in the last 14 days

Tick on the box if the student has been in close contact with the COVID-19 patient

If the applicant has not been in close contact with a case of COVID-19 and was not under mandatory quarantine
Note: For the health and well-being of the community, the NEET exam center authority reserves the right to deny the entry of a candidate.
NEET Admit Card Undertaking 2021: Important Details:

Along with filling the NEET 2021 admit card undertaking, candidates have to affix the passport size photograph as uploaded in the NEET 2021 application form.

A left thumb impression and signature on the NTA undertaking will also be put upon reaching the exam centre in front of the invigilator.

Parents/guardians signature on the NEET 2021 admit card undertaking is mandatory.
NEET 2021 will be conducted in pen and paper-based mode in 13 languages. Through the scores of NEET-UG, admission to 83,075 MBBS, 26,949 BDS, 52,720 AYUSH, and 525 BVSc and AH, 1,899 AIIMS and 249 JIPMER seats will be granted.
